Transition To Middle School, IEP Meeting?

My daughter has her final elementary IEP in two weeks. She will be starting at the middle school this Fall. The problem is I know so little about what to expect in middle school that I am not even sure what to be asking for in this IEP meeting. I know that my number one concern is safety and supervision in changing classes, recess, etc. I worry about wandering, particularly in this brand new, huge school. What are the big issues I need to address with the ARD committee?

Oh also having them put a risk(concerns,dangers)/care (how to prevent and handle) plan into her IEP so her teachers/aides will know what to do.

Maybe ask about the possibility of a one on one para/aide. Someone that could escort her to and from classes, even sit in classes and help her. I have seen this done in a few schools I've been to. It should be available in a large school, with setting/situations that can be over stimulating.
