My son just got a second evaluation done; do we void the 1st one or combine them?

I would just keep it as a matter of record so that you can refer to it when you or the therapists/teachers see changes or if a certain behavior continues and is not being addressed. The teaching and therapy staff would probably go with the most recent evaluation.

In fact, you could create a notebook datewise of any and all evaluations done.
