Teach your kid how to spit. Show what spit means- say it and do it and ask kid to do it. Kid won’t spit the first 500 times but one day it will click and will spit.

We had the same issue. They finally taught her at ABA. I sent toothpaste and brush and they practiced after lunch. Spitting out is counter productive . Really getting more toothpaste in then out . Bad breath spray cover it . Try gum he may not eat that . Do it I front show him you can spit it out .
