The first thing we tried was adding zinc and magnesium supplements. We also went to a Gluten free diet, and we restricted casein to a very low level.
Each of those helped a little.
We also restricted raw sugar, which helped a little. Finally, we increased his daily exercise, which helps a little. Finally, we are very careful with food dyes and vitamins that contain iron or copper.
All of them together made a huge difference. He went from sleeping in two-hour shifts, to nearly eight hours.
Here is the order of things that I think helped most:
1. Chewable magnesium supplements.
2. Zinc Supplements
3. No copper or iron in supplements.
4. Gluten free diet.

I agree with Dave, but I would also add that you should go talk with a holistic doctor or a DAN doctor. They will be able to prescribe supplements specific to your situation pretty quickly based on the issues you face. It has helped us with bedtime as well.
Go up on the Melatonin dose. Also, consider treating for yeast. When yeast levels are high, as well as when you are first treating the yeast and there is die-off from the treatment, the toxins from the yeast cause our kids a lot of problems neurologically.
what about time released melatonin?
My daughter has always had issues with sleep. Her max awake time to date is 52hrs...it was horrible. She is on trazadone, resperidol, and now hydroxyzine at night to replace the melatonin. It works some of the time. We've been battling her sleep for years now and she is only 5. She has a sleep psychologist and we are just praying this will work.
