Go to the Social Security Administration website. You can apply for SSI online. Then you will be contacted regarding verification that's needed. Make sure that you provide good contact information. Good luck.

I think it varies slightly from state to state. In Tennessee, they set you up for an appointment after you turn in the initial paperwork they ask for. If you call them, they can tell you.

Depending on the disability, you would need the diagnosis from the doctor and then some sort of services from the state, for example we have family voices of NH and Community Bridges and special schools in our area so you would have to do some digging and see the options in your area.

The social security office will supply you with forms for SSI. You will need proof of income, assets, bills. and diagnosis.
