Guardianship in certain states is done at 15 and other states at 18 but it’s a lifelong document and the age of the child has no bearing on the document it’s good for the life of the child however you can add other sister or brother to it once you are too old to look after

Yes, I did when my son was 5 years old and then updated it again when he turned 16. He will be 18 in December.
