Hello Everyone It’s been so long since I have been on I have been so busy with My son Larry He is 20 now πŸ˜… I have a question about IHHS protective Supervision I got approved But I need a Doctor SOC 821 filled out Does anyone know how this form should be filled out In order to qualify for PS My son is somewhat verbal ( he is considered severe Autistic) But He needs constant supervision at home at school etc... My Social Worker approved me For hrs But I was told I should get PS for My son She was very nice Thank God 😊 And asked if I could get The Doctor Soc 821 form My son is considered severe He is 20 But has The mentality of a 2&3&4 year old My Granddaughter is 4 And She is much more advanced Then Larry πŸ˜”πŸ˜«πŸ˜“

I don't know the name of the form but I wrote down everything that my children needed and broke it down to the hour because that is how they determine what they are going to give you. How many hours for bathing...feeding etc. If you look on the IHSS website there are blank forms that you can fill out. My doctor refused to fill out the forms without me and the kiddos in attendance. If your son is 20 years old and still needs that level of care then in my opinion he would definitely qualify and I would fight for it. My son is severe and my daughter moderate and they both qualify but it is a constant fight to make sure it happens. I send you hugs and positivity! You can also get an advocate to assist you! :)
