My grandson refuses to go poop on the toilet. He will urinate on the toilet. He holds it in so long and squeezes his cheeks together to keep from going. The doctor has us giving him miralax, but this has been going on for over 3 years and this cannot be healthy.

The hurt compounds the fear. I think mine was afraid of part of himself falling out , the sound, the "unnatural" position, etc etc. Took a long time of mild laxatives, coaxing and (yes) holding him down. Finally after he did it a few times it was ok. But yes, the size! Also, in a diaper in his sleep, in a fetal position, it sometimes "happened" but that wasn't as frightening to him.
I have tried to force him to sit on the toilet when I see him scrunching up in pain, standing on his toes, clinching his little bottom up, and telling me to go away his tummy hurts. He screams and yells at me when I put him on the toilet. I try praising him. I try rubbing his tummy while on the toilet. It just seems to clinch up tighter. I hate to see him in pain daily because of this issue. Be the time he does finally go no wonder it hurts because it is bigger than any 7 foot man would make and as hard as a baseball. It has to hurt.
Started going without issue on a daily basis. He even looks forward to sitting on the potty so he can watch train videos on YouTube. Important to give lots of praise too. Good luck.

Keep making him sit, let him play games, read books, etc. my son would hold it all day and only poop at night after he fell asleep, muscles relaxed and unaware. after months of sittingwith no luck and 3am diaper changes, I went against my 'better judgement'. when he felt the urge to go while on the potty he would clinch and scream in fear.
